About this deal

We found a breach of Regulation 17, Health and Social Care Act 2008 (Regulated Activities) Regulations 2014, Good governance. Sometimes called inpatient treatment, residential addiction treatment requires clients to live at a facility 24/7. During their time in a structured and supervised environment, individuals have the chance to focus solely on recovery. They’ll undergo comprehensive individual and group counseling as they work with professionals to understand their disease. People felt safe staying at Serenity House and risks to people were minimized through risk assessments. There were plans in place for foreseeable emergencies. During Serenity House residential addiction treatment, clients also learn helpful strategies and coping skills for dealing with post-rehab life. They’ll begin to understand how stress and environmental triggers can easily lead to relapse. Additionally, they’ll learn plans for combating these obstacles to recovery. What We Offer This inspection took place on 14 July 2015 and was unannounced. North Warren provides care for older people who have mental and physical health needs including people living with dementia. It provides accommodation for up to 15 people who require personal and nursing care. At the time of our inspection there were 13 people living at the home. The provider also provides personal care to people in their own homes. At the time of our inspection the provider was providing personal care to 42 people.

The provider ensured that all staff received appropriate professional development. People who lived in the home confirmed that staff had the right knowledge and skills to support them. One person told us that since coming to Serenity House they had managed to 'Change my life, turn my life around'. We were told how their relationship with their family had also improved. The other person was equally positive about the service. During the inspection there was an event and notification of a specific incident. This incident is subject to a criminal investigation. As a result, this inspection did not examine the circumstances of the incident. Prior to this visit we made a telephone call to inform the provider that we would be inspecting the service the next day. The reason we announced our visit is because people who use the service lead active lives and we wanted to make sure we would be able to meet one or both of them. During this visit we spoke with one of the two people using the service, the provider and a member of staff.

By getting help at our residential detox center, you’ll see that you don’t have to do this alone. While in treatment, you’ll see that there are many other people dealing with similar issues, and they’ll be there as part of your support system, along with staff and counselors. This will allow you to see that you’re not alone, and you never have to be alone again.

People who used the service told us that staff respected their choices and that they were supported and enabled to do things independently. They confirmed that they were getting the support they needed and told us how they took part in the ongoing assessment and review of their needs and goals. They said they were involved in the recruitment and selection of staff and that staff had the right skills to support them. They told us about how they were encouraged to take part in the daily running of the service. A complaints procedure was in place. There were appropriate management arrangements in place. Staff felt supported by the provider and assistant manager and staff meetings took place. Regular audits of the service were carried out to assess and monitor the quality of the service. We saw that staff obtained people’s consent before providing care to them. Where people could not consent, assessments to ensure decisions were made in people’s best interest had usually been completed.

Patients are cared for in a tranquil and homelike setting. Each private room allows patients to be surrounded by loved ones throughout their time with us. We spoke with the two people using the service and looked at their care and support records. We also spoke with the provider (who is also the manager) and three staff. This is a summary of what we found - The provider acted in accordance with the Mental Capacity Act 2005 (MCA) and Deprivation of Liberty Safeguards (DoLS). The MCA provides the legal framework to assess people’s capacity to make certain decisions, at a certain time. If the location is a care home the Care Quality Commission is required by law to monitor the operation of the DoLS, and to report on what we find. Karen has also set up and managed one of the Caribbean’s premier bespoke treatment centres in Barbados, working tirelessly to develop a treatment programme that combined talking therapy, adventure therapy, restorative medicine and the beauty and peace of nature.

Our retreats aim for those with less challenging issues than those needing our full programmes to reduce feelings of anxiety stress, burn-out and sleep issues through empowering change. The last rating for this service was requires improvement (published 22 January 2020). The service remains rated requires improvement. This service has been rated requires improvement for the last three consecutive inspections. People were provided with a choice of suitable and nutritious food and drink. Staff provided people with different options, including the use of pictorial menus and shopping lists, so that they were able to make an informed choice. People told us about favourite meals that they liked to cook and staff said this was encouraged and supported. The provider sent evidence to demonstrate the urgent action taken to reduce environmental and fire safety risks to keep people safe. We identified breaches in relation to safe care and treatment and good governance at this inspection. Please see the action we have told the provider to take at the end of this report.

Quality monitoring systems were not consistent. Audits did not always identify shortfalls and where shortfalls were identified, action was not always taken to resolve them. The provider had failed to notify us about an incident they were required to by law. There was an open, person centred culture in the service. There had been recent changes in the management team which had a positive impact on sharing a vision for the service. However, people, staff and relatives felt the registered manager was approachable and cared about people. Swear words, sexual references, hate speech, discriminatory remarks, threats, or references to violence Serenity House is registered to provide accommodation for up to three adults who may have complex needs including learning disabilities, mental health and/or substance misuse issues. At the time of our inspection there two people living at the home. The provider of the service was also the manager. The service has two team leaders and six care staff as well as an administration worker. The service had a manager registered with the Care Quality Commission. This means that they and the provider are legally responsible for how the service is run and for the quality and safety of the care provided. People were mostly supported to have maximum choice and control of their lives and staff mostly supported them in the least restrictive way possible and in their best interests. Although policies were in place the systems were not robust to support this practice.
